Now you can join our “Dental Discount Savings Plus” Plan for a nominal membership fee. Our Discount Plan entitles you to preventative, corrective and periodontal dental care at 25% off and your annual check-up exam is FREE. It also includes an additional FREE Emergency exam for yourself or as a gift to someone else. To enroll, simply fill out an application at our office and make your payment with a check, money-order or credit card.

Patients agree that Sierra Gate Family Dental In-House Dental Discount Savings Plus Plan fees must be paid at the time services are provided. Any fees left unpaid upon receiving a service will be billed at REGULAR fees. Discount fees are valid ONLY when paid at the time of enrollment. Discount plans are not transferrable to another person. Discount plans are NOT insurance and may NOT be combined with any other insurance.
Products and lab fees are not included in the plan.
